1/6/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Josiah upon arrival to the home setting. Upon arrival to the home setting, Josiah appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C inquired about his morning. He verbalized to QMHP-C that his morning was going well. QMHP-C engaged Ms. Bates in a discussion to determine if there were any new behaviors since the last session. Ms. Bates reports that Josiah has been getting upset and leaving out of the house when he cannot have his way with behaviors lasting 20 minutes in duration on a daily basis. 2. Behavior: Josiah presented cooperative and normal behaviors as he did not need any redirection from QMHP-C as well as him answering open -ended questions and being praised by QMHP-C. He reports that he gets mad at his mother and leave the home because they âhis mother and grandmotherâ do not let him do what he wants to so he just gets mad and leaves the home as mother reports his behaviors are lasting up to 5-10 minutes in duration. 1.Intervention: QMHP-C reviewed with Josiah the interventions from the previous session and explored with Josiah and his mother Josiahâs positive coping skills for his emotions. QMHP-C engaged Josiah and his mother through a therapeutic activity and encouraged Josiah to utilize his positive coping skills during periods of frustration and anger during the therapeutic activity. 2. Intervention: QMHP-C provided Josiah with reassurance and support throughout the session. QMHP-C ended session by encouraging Josiah to continue addressing his service plan goals. 1.Response: Josiah appeared to be receptive to this counselorsâ interventions and services that was provided to him during the session. While exploring Josiahâs emotional state, Josiah mentioned to QMHP-C that he was in a good mood. Josiah actively participated during the therapeutic activity, evident by him participating in the activity and being active throughout the activity. 2.Response: While exploring positive coping skills, Josiah stated that the coping skills that he was going to continue to use positive coping skills when he gets to the point where he doesnât know what to think or do.
